USA Hockey Forms - Waiver of Liability and Consent to Treat - are now available on the "Register" Menu.  These forms are required to be completed in order to participate as a player or coach on the NJ Dare Devils.  Please print, complete and sign (or have one of your parents sign if under the age of 18); bring the completed form to the next practice.
